At Advantage, we've guided families through this transition hundreds of times across Metro Atlanta. The first week of care at home typically involves a lot more coordination than families anticipate, and a lot more peace of mind than they expect once a routine is established. Before a caregiver ever arrives for a first shift, our care coordinators review the discharge instructions, assess the home environment, and set up a care plan that matches what the client actually needs in the days immediately after a hospital stay.
In practice, that first week often looks like this: medication reminders aligned to the discharge schedule, help with bathing and hygiene while mobility may be limited, meal preparation that accommodates any dietary restrictions from the hospital, and consistent monitoring of how the client is responding to the transition home. Our RN supervisor is available throughout, and our caregivers are W-2-employed CNAs, not contractors picked up on an app.
Post-hospitalization recovery at home works best when the support structure is in place before discharge, not scrambled together the morning someone comes home. We can typically start care within 24 hours of a call.
